How to Register

When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form. These must be completed before you see the Health Care Assistant, so it is advisable to attend at least 10 minutes before your appointment time, so that you can complete the information required.

The receptionist will offer you a new patient appointment these are in the afternoon/early evening.

Acceptance on to the list is as at the discretion of the Doctor. Please note that your medical records may take several weeks to arrive at the Practice.

Once you’ve been accepted as a patient, your medical records will be transferred to the Practice. If you default from your new patient appointment and do not inform the reception staff you may be refused acceptance on to the Practice list.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Register online

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services. All new patients are required to contact The Surgery to make a registration appointment to be seen face to face in the surgery. Your registration is not completed until you have been seen.

If you prefer you can download registration form, via NHS inform, and our New Patient Questionnaire form then upload to the practice below or alternatively complete and bring with you. All new patients are required to contact The Surgery to make a registration appointment to be seen face to face in the surgery. Once you have completed the forms please upload them to the practice.
